| A | New customers need to do the following:
Complete the MTN Service Provider Insurance Application Form. Fax the completed Application form to 011 912 5122. Ensure that
you are issued with a copy of the Terms & Conditions of the policy as well as the Policy Schedule.
When you receive your first MTN Service Provider account, check that your insurance premium is listed as a separate item to ensure that you are in fact covered.
Existing customers to do the following:
Complete a copy of the Insurance Application form. Fax the completed form to MTN Service Provider on 011 912 5122. Please ensure that you receive a copy of the Insurance Terms and Conditions as well as a copy of the Policy Schedule. Click here to access online insurance forms. |
| A | The excess on stolen/ lost handset:
- The first claim is R350.00
- If you claim again within 12 months of previous claim, the excess is R750.00
- If you claim for a 3rd time within this 12 month period, the excess is R1000.00. The insurance Policy will also be terminated after this claim.
- An additional excess of R300.00 will be payable for any claim occurring within 60 days of the inception date of the policy.
The excess on damaged handset:
- the first claim is R150.00
- If you claim again within 12 months of previous claim, the excess is R300.00
- If you claim for a 3rd time within this 12 month period, the excess is R350.00.
- An additional excess of R300.00 will be payable for any claim occurring within 60 days of the inception date of the policy

| A | You will be covered for theft, loss or damage, as well as liquid damage. You are only covered for the phone on which the premiums are paid as long as it is used in conjunction to the stipulated MSISDN (cellular number) as per the application form. |

- If your phone has been lost or stolen, call the Customer Care Helpline on 083 1 808 to suspend your phone.
- Call the insurance department on 083 123 6084 and inform them of the incident. The insurers will then provide you with an ITC reference number, confirming the blacklisting of your handset.
- Report the loss or theft to the police and obtain a police case number.
- Complete an MTN Service Provider insurance claim form and fax the completed form to MTN Service Provider Insurance Department.
